MISSION

IMMEDIATE SUPERVISOR: Staff Supervisor
MINIMUM QUALIFICATIONS: The ability to communicate clearly in oral and written forms in order to provide necessary supports and produce a variety of documentation which meets or exceeds state and agency standards. The ability to provide necessary supports to people with disabilities. Must be a high school graduate or equivalent and be at least eighteen years old.
OTHER QUALIFICATIONS: A valid driver's license and a driving record that is acceptable from a risk management perspective may be required. The ability to physically climb stairs, check under furniture at floor level and lift up to 50 pounds is required. The physical ability to assist in lifting adults may be required. May be required to have or obtain an Iowa Medication Manager Certification. Basic timekeeping and math skills are required.
POSITION SUMMARY: Attends to the personal and programmatic needs of the individuals being served in the service center; accesses community independently to support individuals served during community outings. Responsible for providing the personal assistance and supervision required for individuals served to improve and maintain quality of life as outlined in Individual Service Plans (ISPs).
Primary Responsibilities
- Follow Individual Service Plans to assure excellent service provision.
- Maintain an organized, clean and productive work area and keep supervisor informed of the needs of individuals, self, and co-workers.
- Establishes positive relationships with individuals served as indicated by quality communication, commitment to individual rights and responsibilities, appropriate interactions, and ability to create a balanced relationship with clear boundaries that allows for relating on fundamental levels.
- Responsible for diligent monitoring and reporting of the physical and mental health of individuals served including any health changes and any out of the ordinary occurrences.
- Keeps supervisor informed of important incidents, plans, issues concerning individuals served.
- Acts as an advocate for individuals served.
- Strictly adheres to confidentiality guidelines.
Documentation
- Responsible for writing progress notes for each shift worked that meet or exceed agency and state standards and meet established timelines.
- Responsible for completing all applicable documentation according to agency standards and timelines connected to service provision for all shifts worked.
- Responsible for making all corrections to documentation as directed by supervisor.

SECONDARY JOB DUTIES:
- Assists in the development of the ISP
- May be required to transport and accompany individuals in the community as needed.
- Maintains and/or assists with the financial records for the individual and the work site where applicable.
POSITION AND ENVIRONMENTAL CONTEXT
- The position requires the performance of tasks requiring visual, manual, communication and analytical abilities within a team setting.
- The position requires that the work be performed in the facility with an emphasis on community integration. Therefore, the ability to independently travel extensively in the community and to access a variety of locations is required.
- The position represents the organization in many different forums. Professionalism in dress and conduct as well as appropriate representation of the agency is expected.
