POSITION: STEAP Facilitator
PROGRAM ASSIGNMENT: Employment Systems
IMMEDIATE SUPERVISOR: Director of Employment Systems
MINIMUM QUALIFICATIONS: Demonstrated experience working with people with people with disabilities or similar service needs. Must be 18 years of age and have a high school diploma or equivalent education. A valid driver’s license is also necessary, as well as a driving record that is acceptable to the agency’s insurers.
ESSENTIAL JOB DUTIES:
The STEAP Facilitator will be responsible for overseeing and participating in the planning and implementation of the Summer Transition Employment and Assessment Program. See Appendix A for a full description of the STEAP Program. Each of the parts of this job, as outlined below, will be performed with the support of the Employment Coordinators.
PLANNING
- Involves self in budgeting plans and deals with individual funding for each consumer.
- Attends IEP meetings with the school system.
- Involves and familiarizes self with the residential staff and programming for each consumer.
- Writes individualized plans for each consumer based on the IEP, ISP, and team input.
- Develops staff training program.
- Assists in job development.
PARTNERSHIPS
- Develops relationships with consumers, families, teachers, staff, employers and others to ensure quality and consistent services to all team members.
- Works closely with Employment Administrator to develop job sites.
- Recognizes the different needs of team members, and facilitates compromise and consensus, with the wants and needs of the consumer in mind.
- Works with other area providers as necessary.
IMPLEMENTATION
- Oversees each job site over the summer, maintaining relationships and addressing concerns.
- Ensures that consumer plans are being run and documented with accuracy.
- Takes a strong leadership role in crisis management.
- Maintains accurate attendance records for billing purposes.
- Reviews, at least weekly, documentation of services to discover information and ensure accuracy.
SUPERVISORY
- Supervises all Direct Support Coaches.
- Provides initial and ongoing training to coaches, as necessary.
- Manages staff hours and payroll.
- Takes disciplinary action when called for.
- Facilitates teamwork and initiative among coaches.
- Provides written performance evaluations to all coaches by the final day of programming.
FOLLOW THROUGH
- Writes final assessment for each consumer, based on documentation collected over the summer, and distributes this information to the team.
- Provides input to the “Final Program Report”, through written and verbal summaries.
- Organizes and implements end –of-summer transitions back to school for designated students.
SECONDARY JOB DUTIES
- Transports consumers to and from job sites.
POSITION AND ENVIRONMENTAL VARIABLES
- This position requires extensive travel in the community and contact with many partners of the agency. Professional representation of Systems Unlimited, Inc. in appearance and attitude is expected.
- The ability to problem solve and clearly communicate action items to others, under time constraints, is essential to the performance of this job.
