Service Manager

PROGRAM ASSIGNMENT: Supported Community Living

IMMEDIATE SUPERVISOR: Director, Supported Living

MINIMUM QUALIFICATIONS: Preference of a BA/BS from an accredited college or university and/ or a combination of education and experience working with people with disabilities which totals five years. A Human Service background and experience in service provision is essential. Must be able to read a wide variety of documents, expand or edit those documents and maintain them in an organized manner.

OTHER QUALIFICATIONS: A valid driver's license and a driving record that is acceptable from a risk management perspective may be required. The ability to communicate clearly in oral and written form is essential.

POSITION SUMMARY: Responsible for writing and disseminating all caseload service plans and reports and maintaining case files in accordance with Quality Assurance standards. Responsible for ensuring services are provided as stated in the individual service plan. Acts as an advocate to procure services that uphold the rights of individuals and the Mission and Vision of the agency and constantly looks to improve upon these services. Attends all scheduled service meetings and coordinates trainings specific to caseload.  

PRIMARY RESPONSIBILITIES:

A.  Service Provision 

  1. Coordinates development of individual service plans by attending/convening service meetings.
  2. Works with supported living staffs to ensure service implementation as stated in individual service plan. Attends staff meetings and/or provides on-site training when needed.
  3. Responsible for updating key documents that reflect service/need changes to facilitate timely implementation of current services.
  4. Responsible for writing and distributing all service reports to interdisciplinary team members within established timelines.
  5. Ensures all case files are kept current and maintained in accordance with established standards.
  6. Provides service updates to Case Management when requested.
  7. Ensures service plan implementation by regularly reviewing service documentation.
  8. Performs and documents monthly face-to-face meetings with individuals at their homes to ensure services are still appropriate.
  9. Provides counseling, problem-solving, and mediation for individuals and their ISP teams to resolve conflicts/foster relationships relative to the needs of the individual.
  10. Collaborates with the Quality Assurance Department to ensure Quality Assurance recommendations are implemented in a timely manner.
  11. Acts as main contact for stakeholders.
  12. Works with internal and external parties to facilitate referral process. Works closely with Family Relations Coordinator to ensure all applicable service documents and authorizations are obtained prior to service start date. Works closely with applicable internal departments to ensure implementation of specific transition plan into services.
  13. Utilizes agency resources in addition to external resources/providers to procure and implement best service practices

B.  Financial

  1. Develops individual/site budgets that reflect services outlined in individual service plans.
  2. Secures, processes, and files all applicable funding documents in a timely manner. These documents include waiver budget data sheets and notice of decision for services, in addition to other funding documents that may be required for service provision.
  3. Explores alternative funding options and supports individuals with accessing viable options.
  4. Completes monthly billing by designated deadlines.
  5. Monitors monthly financial statements of caseload and makes adjustments accordingly to ensure sound financial management.
  6. Works with internal and external supports and agencies to expedite filling vacancies on caseload or within agency.
  7. Acts as main contact to the accounting department to resolve any financial/budgetary concerns relating to assigned caseload.

SECONDARY RESPONSIBILITIES:

  1. Participates in the on-call rotation for the supported living department.
POSITION AND ENVIRONMENTAL CONTEXT
  1. Position entails a high degree of accountability and consequence of error. Errors in judgments can result in severe consequences to the agency.
  2. Position represents the organization in many different forums. Professionalism in dress and conduct as well as appropriate representation of the agency is expected.
  3. Certain position requirements occur in individual homes.  Therefore, the ability to travel extensively throughout the community is required.
  4. This position requires communication and interplay with a number of internal and external   stakeholders.  Therefore, professionalism in communication is considered a key component of this position.
  5. Adheres to office etiquette standards and expectations.
  6. This is a full-time salaried exempt position with normal office hours of 8:00am-5:00pm or as agreed upon with supervisor.  The position may require work in the evenings and weekends.
