RESPITE CARE PROVIDER

JOB DESCRIPTION

MISSION STATEMENT: Systems Unlimited, Inc. exists to serve children, adults and families with disabilities and other challenges to achieve their maximum potential and quality of life.

POSITION: Respite Care Provider

PROGRAM ASSIGNMENT: Respite Program

IMMEDIATE SUPERVISOR: Supported Living Coordinator

MINIMUM QUALIFICATIONS:  The ability to perform the work at the location of employment.

GENERAL RESPONSIBLITIES: To provide care and support to developmentally disabled children or adults in their own home or that of the consumer.

SPECIFIC RESPONSIBILITES:

  1. To provide for the safety of the consumer.
  2. Provide the consumer with desirable food, regularly scheduled daily meals in accordance with the consumer's schedule, balanced menus and proper nutrition. Assists the consumer with the implementation of all of these activities.
  3. Ensures that the consumer has access to the home's living areas; i.e., kitchen, dining area, bathrooms, living room, family room, laundry room, etc.
  4. Assists the consumer with developing and/or maintaining typical family and relative relationships according to his or her desires.
  5. Assists in providing a learning experience with respect to responsibilities, self-discipline and skills required in the home and community environment based upon the consumer's ability and desires.

GENERAL EXPECTATIONS:

  1. To follow the directions of those in supervision in their intent as well as specifics.
  2. To read and follow the Agency's policies, procedures and practices.
  3. To immediately notify the appropriate staff person of any information that is essential to the health and welfare of the consumer.
