Counselor

PROGRAM ASSIGNMENT: Supported Community Living

IMMEDIATE SUPERVISOR: Senior Counselor

MINIMUM QUALIFICATIONS: Abilities judged to be adequate for successful provision of necessary supports to people with disabilities.  Counselors working in the Residential Based Supported Community Living program (children’s program) must be at least 18 years of age and have a high school diploma or GED.  The ability to communicate clearly in oral and written forms in order to provide necessary supports and produce a variety of documentation which meets or exceeds state and agency standards.

OTHER QUALIFICATIONS: A valid driver's license and a driving record that is acceptable from a risk management perspective may be required. The physical ability to assist in lifting consumers may be required for some assignments.

POSITION SUMMARY:  Responsible for working directly with individuals served at assigned worksite(s) and providing the personal assistance and supervision required for individuals served to improve and maintain quality of life as outlined in Individual Service Plans (ISPs).  Responsible for actively supporting individuals in all aspects of service provision in their homes and communities.

PRIMARY RESPONSIBILITIES

Service Provision

  1. Maintains up-to-date, working knowledge of all ISPs and all related service documents. Implements all services as outlined in ISPs and related service documents.
  2. Completes required worksite and classroom training according to established agency timelines. Attends staff meetings.
  3. Makes recommendations to improves services based on firsthand knowledge acquired through implementation of ISPs and experience with the individuals served.
  4. Establishes positive relationships with individuals served and with external stakeholders as indicated by quality communication, commitment to individual rights and responsibilities, appropriate interactions, and ability to create a balanced relationship with clear boundaries that allows for relating on fundamental levels.
  5. Responsible for diligent monitoring and reporting of the physical and mental health of individuals served including any health changes and out of the ordinary occurrences.
  6. Keeps supervisor informed of important incidents, plans, issues concerning individuals served.
  7. Completes worksite specific expectations according to training, direction from supervisor, and agency standards.
  8. Acts as an advocate for individuals served.
  9. Strictly adheres to confidentiality guidelines.

Documentation

  1. Responsible for writing progress notes for each shift worked that meet or exceed agency and state standards and meet established timelines.
  2. Responsible for completing all applicable documentation according to agency standards and timelines connected to service provision for all shifts worked.  This includes, but is not limited to: medication administration, finances of individuals served, incident reports, communications with stakeholders, scheduling matters, and shift checklists.
  3. Responsible for making all corrections to documentation as directed by supervisor.

SECONDARY RESPONSIBILITIES:

  1. Attends medical, psychiatric, and dental appointments with consumers as needed.
  2. May be required to transport and accompany consumers in the community as needed.
  3. Maintains and/or assists with the financial records for the consumers and the work site where applicable.

POSITION AND ENVIRONMENTAL CONTEXT

  1. The position requires the performance of tasks requiring visual, manual, communication and analytical abilities within a team setting.
  2. The position requires that the work be performed in the homes of the consumers. Therefore, the ability to independently travel extensively in the community and to access a variety of residences is required.
  3. The position represents the organization in many different forums. Professionalism in dress and conduct as well as appropriate representation of the agency is expected.
  4. The position will likely include working a variety of shifts which may include: early mornings, days, evenings, weekends, and overnight shifts.  The nature of the position dictates schedules from time to time will likely, unexpectedly be changed in response to the ever-changing needs of individuals served and to always assure appropriate supervision and provision of services.
