Family Systems
IMMEDIATE SUPERVISOR:
Director of Family Systems
MINIMUM QUALIFICATIONS:
Bachelor's degree from an accredited College or University in Human Services or a related field and 2 years experience with children; or, graduate level degree Master's of Social Work. A valid driver's license and a driving record acceptable from a risk management perspective are required.

GENERAL DUTIES:
- Consistently attend staff meetings and supervision.
- Meet training requirements as determined by supervisor.
- Document services in a timely manner.
- Meet or exceed productivity guidelines.
- Communicate consistently with supervisor, child, parents, school, etc.
- Establish and maintain professional boundaries in all areas of job responsibilities.
SPECIFIC RESPONSIBILITIES:
- Follow treatment plan developed by diagnostic staff.
- Teach skills to children to address individual medical needs while following plan.
- Document service in medical model to meet Medicaid mode.
- Develop partnerships with community professionals.
- Provide parents with skill building services in order to teach parents how to deal with children's medical needs.
- Support parents in referrals for children and with funding assistance.
- Provide safe environment for children.
- Report concerns for safety of children to DHS according to Mandatory Reporter requirements.
- Turn in expected program documentation in a timely and accurate manner.
- Assist in scheduling and transportation of a child to a diagnostic assessment.
- Collaborate and consult with child and family in order to track baseline and outcomes.
- Implement appropriate intervention and strategies to meet medical needs of child.
- Report changes in child that would result in changes to treatment plan.
- Complete Diagnostic Assessments
- Complete implementation plans to meet IME requirements.
- Complete remedial order and determine appropriate services for the child.
- Collaborate and consult with child, parents and other professionals
- Get releases signed.
- Make appropriate referrals for child
- Complete Social History.
POSITION AND ENVIRONMENTAL
CONTEXT VARIABLES
- The position requires the performance of tasks requiring multiple communication abilities along with the ability to make independent judgments under the pressure of time and circumstance.
- Represents the organization in many different forums. Professionalism in actions and decorum as well as appropriate representation of the agency is expected.
- The position entails a high degree of accountability and consequence of error. Errors in judgment can result in severe consequences to the agency.
- This is a full time salaried exempt position with normal office hours of 8:00 a.m. until 5:00 p.m. However, the nature of the position requires occasional work in the evenings and on weekends.
